HOPE Timepoint Documentation Series
The HOPE Timepoint Documentation Series, was designed to help Hospice Providers educate their staff on how to complete the required HOPE Timepoints for submission. It is the responsibility of Hospice providers to accurately and completely document each HOPE Timepoint. Included in the series, Kathy walks your staff through how to document a HOPE Admission accurately, HOPE Update Visit #1 and #2, and a HOPE Discharge.
Hospice Providers must begin utilizing the HOPE Timepoints on all Hospice Admissions beginning Oct. 1, 2025. Use these tools to get your staff up and running, to come back as a refresher to answer any questions, to onboard new staff or to help any staff in the office who will be verifying HOPE Timepoint Records.
