The HOPE Tool Part 3 webinar provides a comprehensive review of Sections I through Z, including significant updates and new items in Sections I (Active Diagnosis), J (Health Conditions), M (Skin Conditions), N (Medications), and Z (Record Administration). The session emphasizes differences from the HIS tool and includes detailed guidance on item displays, timepoints for completion, item-specific instructions, coding tips, and real-world examples to ensure accurate and compliant data entry in the HOPE tool.

The HOPE Tool Part 2 webinar offers a detailed review of Section A (Administrative), highlighting new data elements added compared to the HIS tool, and Section F (Preferences), providing guidance on item display, completion timepoints, coding instructions, and examples to support accurate HOPE tool data collection.

The HOPE Tool Part 1 webinar provides an overview of data collection timepoints, timeframes, and submission requirements for the HOPE tool, clarifies who is responsible for completing it, outlines general conventions for accurate completion, and explains how it aligns with HQRP compliance standards.

The HOPE Tool will replace HIS effective October 1, 2025. The HOPE Tool requirement is designed to collect patient specific data in real-time based on interactions with patient and family/caregiver, and with flexibility to accommodate patients with varying clinical needs. This bundled series is designed to help Hospice providers better understand the HOPE tool submission requirement.
This includes required timepoints and submission timeframes including dataset items for accurate completion. CMS considers it the responsibility of the Hospice provider to complete the HOPE Tool accurately and completely. The HOPE Tool Bundle series is designed to help Hospice providers meet that goal.
